    If you decide to cancel registration by MAY 10, you will receive a 50% refund. If you decide to cancel registration after MAY 10, there will be no refund. In the event of the camp being cancelled due to severe weather, 75% of the camp fee will be refunded.

I hereby RELEASE, WAIVE, AND COVENANT NOT TO SUE, Team Jackson Kicking, the staff, Ladd-Peebles Stadium and their staff, and the organization hosting the event from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my family. By signing this form, I also will allow Team Jackson Kicking to use any pictures, videos, or interviews on the internet, including social media sites and marketing materials.
