FEF Partnership Grant Application

FEF Partnership Projects:
·      serve an identified need
·      have broad impact
·      feature a partnership with the Falmouth Public Schools
·      include the support/participation of the Superintendent or designee
·      may be submitted outside other FEF grant cycles or deadline

Complete the following, including all requested information. If the grant is awarded, you are asked:
· to complete an end-of-project or a periodic self-assessment
· to credit FEF in any publicity, interviews, and printed materials

I. Project Overview: Please provide an overview of your project.

II. Describe the goal/s. Include a statement about identified need (Why is the project important and what need/s does it address?).

lll. List the project's primary activities. Please Include a probable time table for these activities and projected number of participating students.

IV. Describe the ways success and impact will be gauged.

V. Provide contact information (name, organization, mailing address, email, phone number) of the primary applicant from each organization. Press "save" after filling out the information for each applicant.

Save it to your computer with the name of your project (e.g.,icreate.docx)
Complete the form.
Upload the form to the space below by clicking the "Browse File" button to browse your computer.
Once uploaded, the file name will be displayed below.

All applicants on the proposal must sign electronically below. Use the blue dot to make your signatures.

Once you are finished, click on the Preview link at the bottom of the page to read through your responses. When you are satisfied with your responses, then click Submit from the Preview page. You can print the form at any point by clicking the Print form.

Once you submit the form, you will see a screen that indicates that your form has been submitted. It will include two links: one is to edit your form, and one is to get a PDF of your form.

The edit button lets you edit the form as many times as you need. Each time you enter new information, however, you will need to preview and then submit the form. All your previously entered information will be there, as long as you use the edit link provided on the acknowledgement screen.

You may want to download and save the PDF to your computer and retitle it with the name of your project.
