By signing below, I (meaning myself if I am age 18 or older or as the parent or legal guardian of the minor child participating in Blockwork) acknowledge and accept the risks inherit in performing volunteer work for BlockWork, including being transported to or from any BlockWork site. Further, I hereby release, indemnify, and hold harmless the City of Salisbury, including its officers, directors, employees, agents, organizers, and supervisors of all BlockWork activities (collectively, the “City”), from and against all claims, actions, demands, costs, damages, losses, and/or expenses of any kind whatsoever proximately resulting from my participation in BlockWork, including but not limited to court costs and attorney’s fees, incurred in connection with the defense of said matters. This release shall not include any damage or loss proximately caused by the gross negligence or willful misconduct of the City.
Further, I give the City of Salisbury permission to use any photographs or videos including my image or voice to publicize the event.
The foregoing is limited to the BlockWork event on October 25 and 26, 2024.
