The Pre-Home Assessment Tool (Pre-HAT) must be completed prior to the home visit and Home Assessment Tool (HAT). The Pre-HAT may be completed over the phone or face to face.
- All fields and questions on the Pre-HAT are mandatory.
- Categorization of household residents into groups is required and must account for all household residents. When completing this section you must fill in the boxes left to right to ensure that each person is counted only one time. The total of the boxes must equal the number of household residents. Age of household residents is to be captured at the time the Pre-HAT is completed. Each box must be completed, even if the response is zero.
Examples:
* If there is a 19 year old pregnant woman with special health care needs, she is counted as a pregnant woman and only as a ‘pregnant woman’
* If there is a pregnant 30 year old woman, she is counted only as a ‘pregnant women’
* If there is a 3 month old CSHCN, the infant is counted only as a ‘CSHCN’
* If there is a 25 year old with special health care needs, the adult is counted only as ‘22 years and older’
- Landlord information shall be collected unless there is an extenuating circumstance that would put the family in danger of harm or danger of losing their home when the landlord is contacted with the summary report.
- Each numbered question may have only ONE response.
- The quiz is a verbal quiz to be given to the adult serving as the family representative. The answers and brief explanation are provided on the quiz page for the convenience of the person completing the form. The maximum number correct is 10.
Children With Special Health Care Needs (CSHCN):
CSHCN are infants or children from birth through the 21st year who have special health care needs. CSHCN are children who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition. CSHCN require health and related services of a type or amount that goes beyond that which is required by children generally.
