  • PURPOSE OF REPORT: To help evaluate IBRO event organizer experiences.

    Please note that the form includes required fields (indicated by red asterisks) that you must fill in before moving to the next section. There are a total of six sections. If the required field does not apply to your event, please type "Not Applicable." Thank you for your participation and support of IBRO events.

    The personal information on this form will not be shared publicly. It will only be used for internal program evaluation purposes. If there is any reason we would like to share the form information publicly, we will ask your permission before sharing.
