Please use this form to order your Name Badge.

The badges will come with a magnetic back unless specified. People with pacemakers need to order the badge with a pin. You can order up to two Name Badges on each form. If you need to order more, please complete the form again.

As a welcome to the INWR we offer new members a badge for themselves and one additional person free of charge. Please select "New Member" in the payment option field. You will also be asked to provide your membership initiation date (month/year).

We prefer online payment through the PayPal gateway. You do not need a PayPal account to use the online payment method. You can pay via credit or debit card as well as your checking account online.

Cost is $14.00 per badge and includes shipping. All badges will be shipped to your mailing address.
