    Welcome to the Combine Academy Online Application for Admissions.  Please follow the steps below to apply. If you don't have all the necessary information, you can return to the application at anytime.

    1. Click "Next >" on this page, and enter the information requested by the online forms. Note: Required fields are marked with a red asterisk, and Combine Academy will receive the data exactly as it is entered.  Please be careful of spelling, capitalization, and punctuation.
    2. Enter the necessary payment information (Application Fee: $25.00).

      To be considered for additional Financial Aid, a copy of your 2016 IRS Tax Transcripts (NOT 2016 TAX RETURNS) must be submitted to Combine Academy's Administration/ Management. If currently available, please upload those documents below:

    • PAY STUB SUBMISSION

      To be considered for additional Financial Aid, a copy of your most recent pay stubs, dated within 30 days of the application date, must be submitted to Combine Academy's Administration/Management. If you are paid bi-weekly or twice-monthly, this will be two pay stubs, if you are paid weekly, this will mean we require four pay stubs, and if you are paid monthly, only one will be required.The submitted pay stub must be complete and the full document must be clearly legible. The name of your employer, your name, the pay date, and your deductions (social security, federal income tax, etc.) must be displayed, along with the net pay for the check.     I understand that this applicaiton for admission will not be considered complete until my school transcript, proof of health insurance, official test scores (if applicable), non-refundable application fee and individualized needs documents have been received by Combine Academy LLC. 

    I aknowledge that the information presented on this application is complete, correct and truthful, and I am willing to abide by the rules and regulations set forth by the Combine Basketball LLC. If anything is found to be untrue, Combine Academy LLC reserves the right to revoke any offer of admission and, subsequently, remove the student from the Academy without refund.
