I, the Team Captain, believe that each player listed on my team roster:

1. Has a current USTA membership.
2. Has a current NTRP rating or has a verified self-rating.
3. Has met all eligibility requirements to play in this NTRP Level and League.
4. That all of the information contained on my Team's Roster is accurate.

I have informed all Team Members of their obligation to be bound by the USTA League Regulations that are printed on the Internet (www.usta.com) or available to them from me or the Section office.

Each team member has confirmed to me their consent to be bound by the USTA Regulations USTA/Midwest Section Standing Orders and the District Local Rules, such consent being a pre-requisite to being a player in the USTA Leagues Program.

I understand that if above listed rules are not followed, I and/or my team may be suspended from league play for a period of time to be determined. The League Coordinator adn Grievance Committees will enforce any violation of local league rules.

FAILURE TO REPORT SCORES
USTA League Tennis match scores must be reported by Team Captains, Co-Captains or designated players, within 48 hours of the completion of the match. A representative from BOTH Team Captains, CoCaptains or designated players, must either enter or confirm the match scores within 48 hours. Failure to report the match scores may result in the following consequences and it is within the authority of the League Coordinator to impose or not to impose the penalty based on circumstances:

1. A written warning issued to teams for the first offense.
2. Loss of all matches for all subsequent violations.

Match results would still be used to generate player dynamic ratings.

As a USTA League Captain/Co-Captain, you agree to allow Wisconsin Tennis Association to release your personal contact information to other captains in your same division/level and participating facilities for USTA league purposes.

I have read and understand all of the regulations concerning the team waiver form and the reporting of team scores within 48 hours.
