MEDICAL WAIVER
I (Player 1) am 18 years of age or older or the legal guardian of the player participating in The Sand Club Academy Inc event. I have read this document by The Sand Club Academy Inc, staff and contractors (hereafter, referred to as “Staff”) and I UNDERSTAND IT’S CONTENTS. I acknowledge that there are physical risks involved in playing beach volleyball and risks outside of playing volleyball. Here are some risks but not limited to the following: possible concussion from colliding with a player, pole, flying objects (like a baseball) or surrounding cement; auto accidents in the parking lot; serious injuries (scrapped knees/toes/elbows); dislocation; sprains and strains; dehydration; heat stroke; sunburn; bee stings; ant bites; spider bites; scorpion stings; auto or personal theft; vandalism or damage to your personal property; physical and/or emotional injury caused by reckless or non-intentional actions by another player or spectator; and personal health conditions caused by: dust, sand, irrigation, drinking fountain, shower, bathroom or pest control. Volleyball may be an extreme test for a person with a heart condition and/or mental ailment and it is recommended to consult with a physician before playing beach volleyball or participating in any vendor/contractor services performed at this event. I indemnify the Staff from all legal issues resulting from the risks mentioned above as well as any possible injury resulting on the Mesquite High School premises (parking lot or bathroom). I understand that they have the right NOT to participate during extreme weather conditions and assume all responsibility for all injuries (mainly caused by electrical poles or bursting light lamps in winds/rain and/or heat stroke during high temperatures.) With a full understanding of the potential risks and legal issues, I HEREBY ASSUMES THE RISKS OF PARTICIPATING IN ANY EVENT SCHEDULED BY THE SAND CLUB ACADEMY INC from 12/01/2018 to 12/31/2019. I also approves The Sand Club to publish any photos (and tag) in Instagram, Facebook, Twitter, YouTube and other online social media or print forums and indemnify the Staff of all legal issues resulting from personal online or print publications. I understand that the Staff is not responsible for first aid or treatment of any injuries and that there are NO refunds for forfeits or disqualifications.
