  • Panther Kids Classes are Tuesdays and Thursdays from 15:00 - 15:30 Pm. It's recommended for children between 5 - 13 years. Adult supervision is advised during the classes. Your kid's safety is our priority and is under strict supervision by qualified coaches during the workout.

    • 20% Discount apply to parents who are already CF Taniwha Members. This discount is including siblings.
    • 15% Discount apply to parents who are not members and register more than 1 kid.
    • 0% Discount to parents who are not members and 1 kid.
  • Terms of Agreed Sign-Up Period:

    A minimum sign-up of 3 months are required.

    • Cancellation of Training needs to be done one calendar
    month in advance after the minimum agreed period of 3 Months, failure to Cancel the agreement after the 3 month period will result in an auto renewal of the agreement.

    • Failing to comply with the agreed signed/agreed period
    will result in a 75% cancellation fee of the total outstanding amount.

    • Old Members who intends to join the box again will be charge a re-activation fee of R250 for admin purposes.

    The signed Authority and Mandate refers to our contract as dated as on signature hereof (“the Agreement”). I hereby authorise you to issue and deliver payment instructions to the bank for collection against my above mentioned account at my above mentioned bank (or any other bank or branch to which I may transfer my account) on condition that the sum of such payment instructions will never exceed my obligations as agreed to in the Agreement, and commencing on the commencement date and condition until this Authority and Mandate is terminated by me by giving you notice in writing of no less than 30 ordinary working days which will then be subject to cancellation fees as per the contract signed between yourself and Unifest Trading (PTY), and sent by prepaid registered post or delivered to your address indicated above.

    The individual payment instructions so authorised to be issued must be issued and delivered as follows. On the 1st day (“payment day”) of each and every month commencing on .

    In the event that the payment day falls on a Saturday, Sunday or recognized South African public holiday, the payment day will automatically be the very next ordinary business day. Further, if there
    are insufficient funds in the account to meet the obligation, you are entitled to track my account and re-present the instruction for the payment as soon as sufficient funds are available my account.

    I understand that the withdrawals hereby authorised will be processed through a computerized system provided by the South African Banks and I also understand that details of each withdrawal will be printed on my bank statement. Each transaction will contain a number, which must be included in the said payment instruction and if provided to you should enable you to identify the Agreement. A payment reference is added to this form before the issuing of any payment instructions. I shall not be entitled to any refund of amounts which you have withdrawn while this authority was in force, if such amounts were legally owing to you.

    MANDATE
    I acknowledge that all payment instructions issued by you shall be treated by my/our above mentioned bank as if the instruction had been issued by me personally.

    CANCELLATION
    I agree that although this Authority and Mandate may be cancelled by me, such cancellation will not cancel the Agreement. I shall not be entitled to any refund of amounts which you have withdrawn
    while this authority was in force, if such amounts were legally owing to you.

    ASSIGNMENT
    I/We acknowledge that this Authority and Mandate has been ceded to Netcash (pty) Ltd as per your agreement with Netcash (pty) Ltd, but in the absence of such assignment of the Agreement, this Authority and Mandate will be null and void.

  • SCREENING QUESTIONNAIRE

    If you are between the ages of 15 and 69 and are intending to take part in physical activity or regular sport/exercise and you are new to exercise, the questions below will give an indication as to whether you should consult a doctor before you start. If you are over the age of 69 and you are not used to physical activity, you should consult your doctor first in any case. All information you record on this form will be treated with the utmost confidentiality, it will be stored in a secure place and made available to you at any time.
  • Q1 - 13 If you have answered YES to any of questions listed Q1 to Q13 overleaf; we will require you to obtain written consent
    from your doctor before agreeing to undertake any form of fitness testing, physical activity, training or exercise with you. We suggest you talk to your doctor by phone or in person before you begin becoming more physically active and certainly before you undertake any form of formal fitness assessment. Tell your doctor about this questionnaire and which question(s) you answered yes to.


    You may be able to do any activity you want – as long as you begin slowly and build up gradually, or you may need to restrict
    your activities to those which are safe for you. Talk with your doctor about the kinds of activity you wish to participate in and
    follow his/her advice.


    Q14 – 17: If you have answered YES to 2 or more of the questions listed Q14 to Q17 above, we will, for reasons of safety, restrict any programme of exercise to a moderate intensity unless you obtain written consent from your doctor to exercise at a higher intensity. Moderate intensity being a level at which you are able to comfortably sustain exercise for up to at least 60 minutes (usually not exceeding 65% of your maximal heart rate)


    IMPORTANT NOTE: If you proceed with a programme of physical activity and, during that period, your health changes so that you would subsequently answer YES to any of the above questions, inform me immediately as you may need to change or even suspend your physical activity or exercise plan. If you feel unwell because of a temporary illness such as cold or flu, it is advisable to stop training and recover properly - wait until you are better.

  • General Payment Info
    • EFT Payments for Training needs to be made between the 25th and 1st of each month in advance for each month if debit order is not chosen.
    • Should there be any outstanding fees by the 5th of each month; CrossFit Taniwha reserves the right to deny any further training sessions till all outstanding fee(s) are made.
    • Any outstanding fees which are not paid in full will result in legal action.
    • There will be no refunds once payment(s) has been made.
    • There will be a 10% Annual Increase in all Training Packages.

    Public Holiday(s) and Holiday Season(s) Info
    There will be no Training on Public Holidays, unless agreed upon by CrossFit Taniwha.

    Training and Health
    • The client will be training at his/her own risk.
    • Training is a high risk sport, therefore there are potential risks associated with training. I understand and accept these risks.
    • CrossFit Taniwha will not be held liable for any injuries obtained during or after a training session.
    • CrossFit Taniwha cannot be held liable or responsible for any false information given in connection with the client’s health and Physical condition.
    • It will be required of the client to complete a screening questionnaire form which the physical and health condition of the client will be determined.
    • If for any reason the health or physical condition of the client changes it will be the client’s responsibility to inform CrossFit Taniwha of any changes.
    • The Trainer may request the client to get Medical Approval to exercise should he/she at any time feel that the client is in any Physical or Health danger.

    Membership
    • A minimum contract period of 3 Months must be agreed to on all options available to the client in order for him or her to train at CrossFit Taniwha.

  • 1. In consideration of being allowed to participate in the personal fitness training activities and programs of Unifest Trading (Pty) Ltd t/a CrossFit Taniwha and to the use of it’s facilities, equipment, and services in addition to the payment of any fee or charge, I do hereby forever waive, release, and discharge Unifest Trading (Pty) Ltd t/a CrossFit Taniwha and their officers, agents, employees, representatives, executors, and all others acting on their behalf from any and all claims or liabilities for injuries or damages to my person and/or property, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf, arising out of or connected to the use of any equipment at various sites, including home, provided by and/or recommended by Unifest Trading (Pty) Ltd t/a CrossFit Taniwha.

    2. I have been informed, understand, and am aware that strength, flexibility, and aerobic exercises, including the use of equipment, are potentially hazardous activities. I also have been informed, understand, and am aware that fitness activities involve a risk of injury, including a remote risk of death or serious disability, and that I am voluntarily participating in these activities and using equipment and machinery with the full knowledge, understanding, and appreciation of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury or death.

    3. I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent my participation or use of equipment or machinery. I do hereby acknowledge that I have been informed of the need for a physician’s approval for my participation in the exercise activities, programs, and use of exercise equipment. I also acknowledge that it has been recommended that I have yearly or more frequent physical examinations and consultations with my physician as to physical activity, exercise, and use of exercise equipment. I acknowledge that I have either had a physical examination and have been given my physician’s permission to participate, or that I have decided to participate in the exercise activities, programs, and use of equipment without the approval of my physician and do hereby assume all responsibility for my participation in said activities, programs and use of equipment.

    4. I understand that Unifest Trading (Pty) Ltd t/a CrossFit Taniwha provision and maintenance of an exercise/fitness program for me does not constitute an acknowledgement, representation, or indication of my physiological well being, or medical opinion relating to thereto. I also read and understood the Terms and Conditions pertaining to the Contract Period and Cancellation Procedures.
