    Any inside or outside groups, teams, committees, including scouts, clubs and other school-related activities should request meeting dates/rooms via this form. Please be specific in identifying the group and your requests.

  • Building Usage Request:

  • *Use of Kitchen:  Please provide menu in advance and describe how you will be using the kitchen space.   

  • Fees

  • ALL GROUPS ARE ASKED TO CLEAN UP AFTER USE.  

    School Related Activity/Event:  No charge

    Parish Activity/Event:  $50 per hour (includes fees for additional clean up)

    Non-School, Non-Parish Activity/Event:  $75 per hour (includes fee for additional clean up)

    *Kitchen:  If you plan to use the Kitchen facilities, there is an additional PREPAID charge of $100 sanitation fee.

    UPON APPROVAL, an invoice will be issued (Payable 30 days from Invoice Date). 


  • BUILDING ACCESS

    We ask that one person be designated as the main contact and responsible for building access from beginning to end of each event. The designated person is responsible for pick up and return of building access badge.

  • RULES

  • GENERAL RULES

    • DO NOT PROP OPEN DOORS.
    • NO UNSUPERVISED MINORS WILL BE ALLOWED IN FACILITY AT ANY TIME.
    • No ball dribbling, passing or playing of any kind should ever take place in the gym lobby.
    • No dunking and no hanging on the basketball rims.
    • You are not allowed to use the school’s material or teaching aids.
    • Before leaving the building: Please check the all areas to make sure they are in the same condition in which you found them.

     

    ACCESS BADGE RULES

    • The person in charge of event must be present to unlock the door, if applicable, remain present in the building throughout the event and lock up at the end of the event.
    • If assigned a badge, the badge will gain access to the school for ONLY the time stated above.
    • There is a $20 deposit for the badge which will be returned when the badge is returned.

    Violation of any of the above rules may result in forfeiture of use of the facility. Any damages resulting from the above referenced group will be repaired or replaced at your expense.
