    Thank you for your interest in our high school program. An application is required for all members. Please complete and submit the application within 2 weeks of starting classes. The information gathered here may be used to generate appropriate forms for your signature.
  • You will fill out one form per student.  Once you submit this form, kindly fill it out again for each of your children who will be participating in our high school program.  Thank you.

  • OCMA Photo Release Form

  • I/We,  the parent(s)/legal guardian(s) of the child(ren) listed below, hereby grant permission to OCMA to use any photograph of my/our child(ren) for the OCMA Yearbook, OCMA Promotional brochures and materials, the OCMA Website, Facebook, and in any other official publications without further consideration, and I/We acknowledge OCMA’s right to crop or treat the photograph at its discretion. In addition, I/we give permission for OCMA and its Board of Directors to make, use, copyright and publish news stories, still photographs, and/or video or audio recordings.  I/We also acknowledge that performances might be photographed or videotaped by the media and give my/our permission for my/our child(ren) to be included.

     

    I/We also understand that once my image is posted on OCMA’s website, or Facebook, the image can be downloaded by any computer user.  Therefore, I/We agree to indemnify and hold harmless from any claims the following:

     

    Mr. Hallman, Executive Director, Chris Zuniga, Administrator, Sharon Brown, Controller,  or any other OCMA instructor, OCMA Board of Directors, OCMA Website Designers,  Parents of the other students, OCMA Photographers, (both amateur and professional) Any practice or performance location.

     

    NOTE:  OCMA will post a warning indicating that the photos are the property of OCMA and should not be used without written permission.

     

    Photographer release form:

    Any photograph (digital or print) or any video/audio presentation submitted by any person named on this form may be used by OCMA for the OCMA Yearbook, OCMA Promotional brochures and materials, the OCMA Website, Facebook, and in any other official publications without further consideration, and I/We acknowledge OCMA’s right to crop or treat the photographs and edit video or audio submissions at its discretion.
