• Maintenance of Certification Request to Decline MOC Participation
  • Purpose Of This Form
  • Completion of this form is the initial step to discontinue MOC participation for one or more certificates.
     
  • Effects of Declining MOC Participation
     
  • 1. Declining to participate in MOC will immediately effect the MOC Public Reporting status as displayed on the ABR and ABMS website for the certificate(s) you have selected. 
     
    2. ABMS updates are made 3 business days after the change is reported ABMS. 3. The certificate(s) for which you are declining MOC participation with a valid through date will be honored through the date indicated on the certificate(s), even with your request to decline MOC participation. 4. If you are declining MOC participation for a lifetime certificate, that certificate remains active and your MOC Public Reporting Status will be displayed as “Not Required to Participate” in MOC on the ABR and ABMS websites.
     
  • Once you have declined MOC participation, resuming participation will require MOC Re-Enrollment.
