USAPL Policy of Limited Use of Its Logo and Intellectual Property and Agreement:
It is strictly forbidden to use the USAPL logo or any of its intellectual property without completion of this form and without USAPL’s express written permission. Further, if written permission is granted, then that permission extends only to the limited parameters that are accepted and delineated by the USAPL. By signing this Application and inconsideration of being granted the permission of limited use of the USAPL logo and/or intellectual property, you agree to the limited parameters of logo and intellectual property use, if granted. Under no circumstances shall the USAPL logo be used for personal gain or contrary to the goals, mission statement, and policies of the USAPL, or beyond the usage parameters set forth by USAPL. Lastly, USAPL reserves the right to terminate its approval of the use of its Logo or any of its intellectual property at any time or for any reason, and the undersigned agrees to cease and desist using such logo or intellectual property belonging to USAPL immediately. Permission is not granted until you receive this form returned to you with the appropriate official signature of an USAPL officer.

I will follow the guideline of the USAPL Intellectual Property Use Policy. I may not distrubute the logo to any other party outside of this agreement.
