  • I am requesting an "Initial Attendance" I-20.

    Initial Attendance: Select this form for any F/M student who plans to enter the United States to begin study at your school, if you

    • Are applying for F visa at the US visa consulate.
    • Are NOT transferring a SEVIS record to your school from another U.S.   school.
    • Are NOT applying for a change of status in the United States.

    These students generally reside overseas and use the Form I-20 for visa issuance and initial entry through a Department of Homeland Security (DHS) port of entry.

    Please fill out all of the following information and attach all of the requested documents. Remember that the red asterisk (*) next to a field means that it is required.

    For efficient processing of your request, please:

    • Submit all documents in English or with a true English translation made from the original document written in the foreign language. This can be done either by a professional translation done through a licensed translation service, or can be done by someone who is competent in both the foreign language and in English.  In the case of the latter, the person should certify this with the following language:
      • "I certify that I am competent to read and translate in and in English, and I certify that the translation performed above is a true translation taken from the original document written in." NAME, ADDRESS, DATE
    • Submit all documents in PDF format.
    • Include your ITU Student ID Number in the title of all submitted documents
  • Please provide your full name as it appears in your passport.

  • If you have ever been granted OPT after completing a Master's degree in the US, you will not be eligible to apply for CPT during your ITU Master's program.

    If you have dependents, you must provide proof of additional funding for each dependent. Add $15,000 for each dependent. Do not include dependents who were born in the US or who hold US passports. The fees listed in this table are effective Fall 2019 trimester.

  • IMPORTANT: The following types of funding are NOT accepted:

    • Documents older than 6 months from the start of the trimester
    • Income or salary statements 
    • Funds that are not immediately accessible
    • Assets that are not in liquidated form (house, car, etc.)
    • Life insurance policy statements
    • Pension funds
    • Tax return forms or documents
    • Stocks and investment statements
