Alta Vista Boarding & Grooming, LLC will perform a temperament test on all dogs prior to introduction into play group. The temperament test allows the opportunity for a dog to be slowly introduced to a play group while under the close supervision of a play group attendant. Our staff will make every effort to place dogs in appropriate groups according to temperament, energy level, size, and age. Alta Vista Boarding & Grooming, LLC reserves the right to refuse group playtime to any dog demonstrating aggressive behavior toward staff or other dogs. We do understand however, like people, dogs have bad days and may not get along with particular dogs. Consequently, our staff may remove your pet from the play group for a “break” or offer individual playtime.

Alta Vista Boarding & Grooming, LLC requires all vaccinations to be current 48 hours prior to play group participation. The required vaccinations include Rabies, Da2PP/CCV, Bordetella and a topical flea and tick treatment (proof of vaccination is required prior to admittance to playgroup). Spay or neuter is required for all dogs six months of age or older.

Alta Vista Boarding & Grooming, LLC will practice diligence to ensure the safety, well-being, and happiness of every dog in a playgroup. However, there are certain risks associated with allowing dogs to freely interact with other dogs. Even with constant supervision, nips, scratches, muscle soreness, disease, sickness, and dog fights may occur. Each pet owner must understand the associated risks and any veterinary charges are the responsibility of the pet owner.

By signing below, I understand and agree to the terms stated in the aforementioned paragraphs. I understand the risks associated and I authorize Alta Vista Boarding & Grooming, LLC staff to seek medical attention if necessary, and agree to pay for any and all fees incurred. My signature also releases Alta Vista Boarding & Grooming, LLC and its staff from any and all liability for any injury or damage that arises from, but not limited to, actions from another dog during group play.

This agreement and its terms bond a relationship between Alta Vista Boarding & Grooming, LLC and yourself. Each time your pet enters Alta Vista Boarding & Grooming, LLC you affirm the terms of this agreement to indemnify and hold Alta Vista Boarding & Grooming, LLC and its staff harmless from all liability and claims.
