Complete and submit this form at least 2 days before scheduling a pick-up with 1-800-GOT-JUNK. This gives the Inventory Control team time to research all of Q-numbers and confirm that they can be scrapped. Before the units are picked up, remove the Q-number label and any other Quench labels/stickers.
