Dog Adoption Application

This application is to help determine if the proposed adoption is in the best interest of both the dog and your family. Applications for adoption are subject to approval by the rescue staff. Approval for a meeting does NOT guaruntee adoption.

Must be 21 years of age or older to adopt.

Dogs will only be adopted to families in the STL metro area or within 50 miles of Alton, Illinois

Dogs and cats currently in the home MUST have consistent vetting which includes vaccines, heartworm prevention, and your other pets must be spayed/neutered. MONTHLY heartworm prevention year round is required for all current dogs and for all dogs adopted from Found A Hound Rescue. We WILL be calling your veterinarian.

I/We, the undersigned adopting party understand and agree to the following terms and conditions of this contract in the acquisition of the adopted animal hereinafter referred to as "pet":
1. To agree to return the pet to Found a Hound Rescue if unable to keep the pet. To never abandon the pet, give to another person or surrender,, or give the pet to another shelter, pet dealer or laboratory.

2. To agree to notify Found a Hound if the pet is lost or stolen, and to make all reasonable efforts to locate the pet.

3. To agree to notify Found a Hound in the event the pet suffers life threatening illness or injury, or if the pet is deceased.

4. To never allow this pet to be trained for or used for protection or as an attack pet.

5. To treat this pet as a family pet with affection and kindness, never subjecting this pet to neglect, abuse or cruelty.

6. To provide regular veterinary care including annual vaccinations, heartworm preventative, and parasite check.

7. To always keep this pet within a fenced yard - not on a chain or other restraint unless being walked, or safely within the home.

8. To never leave the pet unattended in a yard - fenced or unfenced.

9. To never transport this pet unconfined in an open vehicle (i.e. the bed of a truck).

10. To regularly check the tightness of the pet's collar. To affix any license and tags to the collar and to ever leave a choke chain on a pet when unattended. The Adopter(s) shall comply with all state/local ordinances regarding pet licensing, identification, and restraint.

13. That any pet not full vaccinated, spayed, or neutered priot to adoption must have vetting completed through Found a Hound Rescue at FAHR's veterinarian. There will be no additional charge to adopter.

I/We further understand and agree that:
1. Non-compliance with any of the above terms and conditions may result in the immediate forfeit of the pet and that Found a Hound Rescue may reclaim this pet without payment or compensation to me/us

2. Found a Hound Rescue has advised me/us the pet can sometimes dig, chew, soil, and exhibit other undesirable traits unless properly supervised, confined, and trained.

3. Found a Hound Rescue has advised me that this pet may have been neglected, mistreated, or abused and that special care or precautions may be required.

4. I am also aware that if, for any reason, I am unable to keep this pet, whether voluntary or involuntary, I will not receive a full or partial refund of my adoption fee.

In consideration of the adoption of this pet, I/we agree to assume full responsibility for the conduct of this pet upon the date of which I/we take the pet home. I/We further agree to hold harmless Found a Hound Rescue and any individuals associated with Found a Hound Rescue and to indemnify such organizations or individuals for any damages or costs resulting from liability known or unknown, anticipated or unanticipated, as a result of the pets conduct after the date of this agreement. I/We agree to accept responsibility for any future costs incurred for treatment or prevention of any illness in the pet. I/We understand the pet may have been exposed to infectious diseases.
