Representatives of applicant organizations may use this form to report incorrect information presented by a panelist during the panel review.
An example of incorrect information would be a panelist misstating the number of performances detailed in an application. This is not an opportunity to provide additional information absent from the application.
To ensure that all applicants receive a fair assessment, the Panel Chair will determine if the information on this form is relevant and should be shared with the panel. While we recommend submitting corrections as close to the time of the application's review as possible, submissions will be accepted for consideration until the close of panel.
