This form must be filled out and submitted to the FRVTA State Office, for each contracted display, prior to exhibitor staging. Without this information on file, your company WILL NOT be allowed to stage and/or set their display.

The authorized representative who is named below is responsible for your company display before/during and after show hours, if needed. This person will serve as the liaison between your company and Show Management. However, in the case that the Authorized Representative can not be reached, please provide a secondary contact that will serve as a secondary liaison.

SECONDARY CONTACT: In case of an emergency or problem that may occur within your exhibit(s) in which we cannot reach the Authorized Representative, this person will be responsible to make any decisions on the company’s behalf for purposes of Show exhibit.
