  • If you answered yes to any of these questions, Alaska Training Room recommends you have your doctor sign a release form and give our instructor any specific limitation requirements for your exercise program.

  • If you answered yes to any of these questions yet choose to waive the recommendation to obtain a release from a doctor prior to starting an activity program, please type your name below.

  • If you answered no to all questions, you can be reasonably certain that you can start a new physical activity program. Always notify your instructor immediately if your current health status changes. If you are now, or become pregnant in the future, discuss your exercise program with your Doctor, and notify your instructor immediately so we may recommend certain modifications.

  • Facility Use Agreement:

    No food or drink, other than bottled water, in the facility.

    Indoor-only shoes and shirts are required.

    Please bring a water bottle and towel to workout.

    Space is available to place your coats and personal items but we are not responsible for their safety. Please leave valuables in your locked car.

    Childcare is not available at this time. Please do not bring children to workout unless they are active participants (in an age-appropriate class) and registered members.

    Please sign in at the desk for each workout you attend.

    Guests: Drop-ins are not allowed. Guests are welcome to "try a class" provided they pre-register online (this excludes Boot Camp classes on Saturday/Sunday). New members must complete three weekday classes (we recommend Adult Intro Classes) prior to participation in any "Boot Camp." A 3-class pass is available for short-term guests.

    PLEASE READ: Due to the nature of group circuit training classes and the importance of a proper warm-up, there is a strict tardiness-no admittance policy. If members arrive after a class has started, they may not participate in that class.

  • Release and Waiver/Indemnification

    In consideration of being allowed to participate in fitness related activities at Alaska Training Room, the undersigned acknowledges, appreciates and agrees that:
  • 1. The risk of injury from fitness related activities is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

  • 2. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation; and,

  • 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusually significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of my fitness coach immediately; and,

  • 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Alaska Training  Room and their employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct fitness related activities ("Releasees"), with respect to all and any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
