    Each applicant should provide:

    One pastoral reference: This can be done by a senior pastor, a church elder, an associate pastor, a small group leader, or any other approved church leader who can speak about the candidate's spiritual life.

    One personal reference: This can be from a friend but not a relative.

    One professional reference: This can be from a boss, supervisor, or any person that you've worked under.

    Each of your references must fill out and submit this form.
