I understand that I must read and abide by the Test Accommodation Policies and Guidelines to receive test accommodation services through Student Accessibility Services.I understand that I am responsible for providing my professor of record with a copy of my Accommodation Letter with timely notice for each course in order to use my approved test accommodations.I understand that I am required to gain professor approval of my Onsite Test Accommodation Request Form should I wish to use my approved test accommodations for onsite quizzes/exams.I understand that a complete Onsite Test Accommodation Request Form with professor approval is required to be submitted to accommodations@nu.edu at least 5 business days’ before my onsite quiz/exam.I understand the exam time for my onsite quiz/exam confirmed by SAS begins promptly as scheduled. The Test Accommodation Policies and Guidelines state if the student has not arrived at the testing location within five minutes of the scheduled time he/she may 1) need to reschedule the exam for an alternate date/time or 2) will have the remaining time to complete the exam that day; the Academic Accommodations Coordinator will make the final decision in consultation with the instructor of record.I understand that all disability-related reasons for late arrivals should be brought to the Accessibility Services Coordinator. If it is determined that the exam needs to be rescheduled for an alternate date/time, it is the student’s responsibility to first communicate with his/her professor of record to reschedule the exam.I understand that make-up exams follow the policy as indicated on the individual course syllabus and require professor approval prior to rescheduling test accommodations with SAS for onsite exams.I understand that if I am approved for additional test accommodations, in addition to the extended time, I am required to fill out an Onsite Test Accommodation Request Form and include the specific accommodation I am requesting on the form. This is required for onsite quizzes and exams only.I understand that in order to remain eligible for services, I am required to contact SAS at least 24 hours prior should I wish to cancel my scheduled onsite test accommodations for any reason.I understand that for any electronic quizzes/exams in a Blackboard course, my approved test time extension is NOT automatically applied.I also understand that I am responsible for informing and reminding my professor of record with timely notice to adjust the test time for my quizzes/exams.I understand that I may refer my professor of record to the CIL Faculty Concierge team at 1-877-533-4733 Option 2 as the resource in how to change or adjust my test time in Blackboard.I understand that if I have any questions about test accommodations, my first point of contact is the Academic Accommodations Coordinator at accommodations@nu.edu.
