• Muskego Scholastic Chess Club Registration 2015-2016

  • PLEASE READ THE FOLLOWING BEFORE CLICKING SUBMIT!!

    1) We ask that you please fill out a separate form for every student you would like to register for the Muskego Scholastic Chess Club.  

    2) Note that there is a fee of $25 per student, and a $50 maximum for a family. This will be collected on the first class you attend.  All funds go to the Muskego Scholastic Chess Club and are used to pay for materials, supplies, instructional materials, and paid instructors.  

    3) When you sign below using your mouse or stylus, you are agreeing to the MSCC Code of Conduct shown below.  This Code will be emailed to you for your reference.

  • MSCC CODE OF CONDUCT:

    As a parent of a Muskego student I agree to the following:

    - I hereby give my permission for the above registered student to practice and compete and represent the school in MSCC, USCF, WSCF, and other chess events.

    - I have spoken to the student I have registered herein, and have explained this agreement, and all associated rules and responsibilities of the Muskego Scholastic Chess Club (MSCC).

    - The student I have registered herein understands these rules and responsbilities.

    Rules and Responsibilities:

    - The student is attending a Muskego school within the 2015-16 school year.

    - The student will not be allowed to practice or participate in a tournament if he/she has not been in attendance 3 consecutive hours of any school day because of illness or unexcused absence.

    - The student has adequate insurance coverage, and either I or my insurance plan will cover the cost of any injuries incurred in participation of MSCC activities.

    - If I provide a ride to a child other than my own to/from a chess activity, I agree to obtain explicit permission from a parent/guardian of that child prior to transporting the child to/from the chess event.

    - All students involved with the MSCC agree to maintain their grades - there must be no more than one failing grade during every marking period.

    - No individual student participant shall be permitted to be in possession of, or use alcohol beverages, tobacco,; nor shall they be in possession, involved in the sale, or use any illegal drugs and or drug-related paraphernalia; nor shall they be involved in any type of unacceptable conduct.

    - The student will participate actively and intently in MSCC classes and events, and will not be a distraction to others attempting to learn, practice, or play chess.
