Before you begin, please make sure to have a copy/scan of the documents you have recieved from the Department of Defense or the Veterans Administration (PEB or service connected rating document). If you have been discharged from service please provide your DD214 that shows category of discharge as well. If you are still on active duty, a scanned copy of your Military ID will suffice. Your disability rating should be bri=oken down by category as well, not simply a page that shows what your overall rating is. We need to know what your rating(s) is/are for in order to best determine how to assist.
NOTE: We cannot process your request without these documents. - If you don't have them now, please come back to fill this application when you do as incomplete applications will be deleted from the system.
By sharing this information you give The Independence Fund the right to share your provided information within our secure Care Management System amongst our Care Management Team in order to most expeditiously process your application. The Independence Fund WILL NOT share any information pertaining to our veteran applicants outside of The Independence Fund's internal, secure network without the consent of the veteran applicant.
