Instructions:
1. Complete the following information, electronically sign and then click the "Submit" button at the bottom of the form.
2. Only juniors and seniors may employ this option for a free elective course.
3. Students who chooses to take a course under the Pass/Fail Option must have a minimum of 12 additional credits from courses graded with standard letter grades to be considered for Dean’s List.
4. Once the Pass/Fail option has been exercised, it cannot be retracted.
