I give my permission to The Andy Warhol Museum (AWM), a museum of Carnegie Institute, to photograph my child during their participation in all programs and activities related to youth invasion 2013. I understand that the photographs may be used for archival, educational or promotional purposes, which AWM deems appropriate. For privacy and security reasons the AWM does not include students’ names with photographs when published.
